  • Insurance or Medicaid coverage to ensure seamless service coordination and maximize your benefits.

  • Referring physician's and your receiving healthcare provider's details for care coordination.

  • Medical reason for why the transportation is needed, this may be necessary to get insurance approval.

To speed up the approval process Medi-Cal beneficiaries can download our non-emergency medical transportation justification form to be completion by your physician. After completion your physician may transmit it to our office by fax or mail as listed on the Rapid Response Justification for Non-Emergency Medical Transportation form.

Medicaid Recipients: Please note that being a Medicaid recipient does not qualify you to automatically receive a medical transportation benefit. Medicaid will pay for the most medically appropriate and cost-effective level of transportation to and from services covered by the Medicaid program. However all non-emergency transportation must be authorized prior to scheduling.

To help ease this burden, we accept most major health insurance plans. However, it is the patient's responsibility to verify coverage with their insurance provider and obtain any necessary authorization before using our services, as not all requests may be covered.
